High School Parents:

Winter Registration Will Be Open From September 1 to November 1, 2024

 


The registration portal will open on September 1, 2024.  Once it is closed, there will be no way to register your student for winter sports.  We understand that you may not be able to get a new physical before this time.  However, it is important that you still complete and submit the online forms before November 1, 2024.  In this case, when it asks you to upload your physical paperwork, please upload a piece of paper with the date of your physical appointment so we can make note of it in your child's account. 

If your child has gotten a physical AFTER you have completed registration, please can email it to vschirmer@hammontonps.org to be uploaded into your portal.  DO NOT log back into your completed registration and upload your child's physical yourself.  We will not be notified of this change and it will affect your child being cleared on time.

You must have completed and submitted registration prior to sending the new physical. 

Physicals sent prior to registration completion will be returned.

You do not have to send a copy of your child's physical if you have already uploaded it into the portal.

To expedite medical clearance, please make sure the doctor has filled out all required fields and signed the form.

*We cannot guarantee that physicals submitted after the due date will be cleared in time for the season's first practices.*

 

Middle School Parents:


Winter Registration for HMS Will Be Open From September 25 to November 15, 2024

The registration portal will open on September 25, 2024. Once it is closed after November 15, 2024, there will be no way to register your student for winter sports. We understand that you may not be able to get a new physical before this time. However, it is important that you still complete and submit the online forms before November 15, 2024. In this case, when it asks you to upload your physical paperwork, please upload a piece of paper with the date of your physical appointment so we can make note of it in your child's account.

If your child has gotten a physical AFTER you have completed registration, you can email it to kscavo@hammontonps.org to be uploaded into your portal.

You must have completed the registration process prior to sending the new physical.

All Parents:

Sports physicals completed before July 1, 2024: To expedite medical clearance, please make sure the doctor has signed and stamped both pages and fills out all fields, such as pulse, vision, etc.  The Physical Examination Form and Clearance Form are both required.

Sports physicals completed on or after July 1: Please use the new sports physical forms which includes the Medical Eligibility Form.  Only the PPE Clearance Form is required.

Physical Examinations
 
Students registering for athletics must have a current physical examination on file in the athletic office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the athletic office no later than 3 school days prior to the first day of practice/tryouts.
 
Physicals are valid for 12 months from the date of the last exam.
