High School Parents:

Fall Registration Will Be Open From June 1, 2025 to August 1, 2025


The registration portal will open on June 1, 2025. Once it is closed, there will be no way to register your student for fall sports. We understand that you may not be able to get a new physical before this time. However, it is important that you still complete and submit the online forms before August 1, 2025. In this case, when it asks you to upload your physical paperwork, please upload a piece of paper with the date of your physical appointment so we can note it in your child's account. 

  • If your child has gotten a physical AFTER you have completed registration, please email it to vschirmer@hammontonps.org to be uploaded into your portal. 
  • DO NOT log back into your completed registration and upload your child's physical yourself. We will not be notified of this change, which could affect your child's timely clearance.
  • You must have completed and submitted registration before sending the new physical.
  • Physicals sent before registration completion will be returned.
  • You do not have to send a copy of your child's physical if you have already uploaded it into the portal.

Please ensure the doctor has filled out all required fields and signed the form to expedite medical clearance.

*We cannot guarantee that physicals submitted after the due date will be cleared in time for the season's first practices.*

 

Middle School Parents:

Registration for Spring Sports Will Be Open From February 7, 2025 to March 17, 2025

 


The registration portal will open for spring sports on February 7, 2025. Once it is closed after March 17, 2025, there will be no way to register your student for spring sports. We understand that you may not be able to get a new physical before this time. However, it is important that you still complete and submit the online forms before March 17, 2025. In this case, when it asks you to upload your physical paperwork, please upload a piece of paper with the date of your physical appointment so we can note it in your child's account.

  • If your child gets a physical after you complete registration, you can email it to kscavo@hammontonps.org to upload it to your portal.
  • You must have completed the registration process before sending the new physical.
  • Physicals sent before registration completion will be returned.
  • You do not have to send a copy of your child's physical if you have already uploaded it into the portal.

All Parents:

Sports physicals completed before July 1, 2024: To expedite medical clearance, please make sure the doctor has signed and stamped both pages and filled out all fields, such as pulse, vision, etc. The Physical Examination Form and Clearance Form are both required.

Sports physicals completed on or after July 1, 2024: Please use the new sports physical forms, which include the Medical Eligibility Form. Only the PPE Clearance Form is required.

Physicals are valid for 12 months from the date of the last exam.

Athletic Team Registration

 

Physical Examinations:
 

Students registering for athletics must have a current physical examination on file in the athletic office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the athletic office no later than three school days before the first day of practice/tryouts.
 
Physicals are valid for 12 months from the date of the last exam.
